2016 Partnership Promise Requirements

Everyone enrolled in the Partnership PPO must take action in 2016 by completing the Partnership Promise. 


Complete the online Healthways Well-Being Assessment™ (WBA) by March 15, 2016


Complete a biometric health screening by July 15, 2016

    ► Complete the screening at an onsite screening event

    ► With your physician (Healthways will accept screening results from a doctor's visit [annual physical] between July 16, 2015, and July 15, 2016)

Actively participate in coaching if you are called

    ► Coaching could include a tobacco cessation program and/or case management, which is managed by BlueCross BlueShield, Cigna and Magellan

Keep your contact information current with your employer; or, if a covered spouse, keep your contact information current with Healthways, if it changes.

Below, see a chart with some helpful definitions that pertain to primary health insurance. For more detailed information, member handbooks are available on the Benefits Administration website.

Annual Deductible

All options include an annual deductible. This is the amount you pay out of your pocket before the plan pays for hospital charges and other services that require coinsurance.


Some services require that you pay coinsurance after you meet a deductible. Coinsurance is a percentage of the total cost.


Some services require that you pay a copay (instead of a deductible and coinsurance). A copay is a flat dollar amount, like $25 a visit.

Out-of-Pocket Maximum

The out-of-pocket maximum is the most you will pay for your copays and coinsurance each year. Once you reach your out-of-pocket maximum, the plan pays 100% of covered medical expenses.

In-Network vs.
Out-of-Network Providers

You can see any doctor or go to any healthcare facility you want. However, if you use an “in-network” provider, you will always pay less. That’s because an in-network provider agrees to provide services to our members at discounted rates. Broad networks of doctors and hospitals will continue to be available.

Network Options

Weakley County has two insurance carriers, BlueCross BlueShield and Cigna. The networks for each:
► BlueCross BlueShield of Tennessee: Network S
► Cigna: LocalPlus

Both carriers have providers available throughout Tennessee. Doctors and facilities move in and out of networks from time to time. Check the networks carefully for your preferred doctor or hospital when making your selection.
